Public Health Nurses provide comprehensive health assessments and care plans for older and disabled adults and children served by CSOA. Program participants must be currently enrolled in CSOA services, or have a concurrent referral made to another CSOA program in order to receive services.
The PHN also provides referrals to programs that support people in their home and assistance in obtaining: adaptive medical equipment incontinent supplies other durable medical equipment for their clients
